Person in calm, relaxed state representing gut-directed hypnotherapy for IBS

A landmark randomised controlled trial published in the prestigious journal Gut in July 2026 has added significant new weight to the case for hypnotherapy as a frontline treatment for Irritable Bowel Syndrome — and for the first time, it directly compared an in-person therapist approach with a smartphone-delivered, self-guided hypnotherapy programme.

The results are striking, and for anyone living with IBS, they offer real cause for optimism.

What the Study Found

The trial — conducted across multiple centres and published in Gut (BMJ) — enrolled 230 adults aged 16 to 75 who met the Rome IV diagnostic criteria for IBS. Participants were randomly assigned to one of three 12-week interventions:

  • In-person, therapist-delivered hypnotherapy
  • Smartphone-based, self-guided hypnotherapy
  • Online psychoeducation (the control group)

The primary outcome was an FDA-defined abdominal pain response — meaning at least a 30% reduction in weekly average worst daily pain across at least two of four follow-up weeks. At the 16-week mark, the results were clear:

  • 48% of in-person hypnotherapy patients achieved a meaningful pain response
  • 33% of smartphone-based hypnotherapy patients achieved a pain response
  • 22% of the psychoeducation control group achieved a pain response

Crucially, both hypnotherapy groups maintained their advantage over the control group at the six-month follow-up — suggesting the benefits of hypnotherapy are durable, not just short-lived improvements during treatment.

Why This Study Matters

IBS affects an estimated one in ten people globally, causing chronic abdominal pain, bloating, constipation, and diarrhoea. Despite its prevalence, effective, drug-free treatments remain limited — and many people cycle through medications, dietary restrictions, and specialist appointments with only partial relief.

This is why the gut-brain axis has attracted so much research attention. IBS is now understood not purely as a gastrointestinal disorder, but as a condition deeply shaped by the nervous system, stress responses, and the unconscious mind's influence over the gut. Gut-directed hypnotherapy works by calming this axis — helping the brain send different signals to the gut, reducing hypersensitivity, and improving the body's tolerance of internal sensations.

What makes this particular trial especially significant is the inclusion of the smartphone arm. Previous hypnotherapy research has largely focused on clinic-based, therapist-led sessions. This study demonstrates that a self-guided, app-based approach can still deliver clinically meaningful results — expanding access to a treatment that has previously been available only to those who could find (and afford) a specialist hypnotherapist.

The researchers themselves highlighted the smartphone programme as a "practical, accessible, and cost-effective treatment option" — particularly valuable for patients with limited access to certified therapists or those who prefer flexible, home-based care.

The Gut-Brain Connection: What Hypnotherapy Actually Does

During a gut-directed hypnotherapy session, the listener is guided into a state of deep, focused relaxation. In this state, the unconscious mind becomes more receptive to suggestions that change how the nervous system perceives and responds to gut sensations. Over time, the brain essentially learns to "turn down the volume" on signals that would otherwise trigger pain and discomfort.

This neurological retraining is what produces lasting change. It's not symptom suppression — it's addressing the underlying hypersensitivity of the gut-brain relationship that drives IBS symptoms in the first place.

Could hypnotherapy calm your IBS symptoms?

A 2026 RCT published in Gut found that app-based hypnotherapy significantly reduced IBS abdominal pain compared to no treatment — and the benefits lasted at six months. Clear Minds gives you access to the same style of gut-directed, mind-calming sessions from your phone, with a full free trial so you can experience the difference yourself.
